Clinical Case Coordinator (2 positions)

Humana's Hunter’s House is an innovative mental health program providing meaningful and evidence based/informed live-in intensive treatment to young people in transition. This short-term assessment and stabilization program is open for people aged 12-18 who experience complex mental health challenges and require a small group care setting to stabilize. Stabilization will focus on meeting each individual’s basic needs, as well as getting proper psychiatric and psychological care to support long-term treatment for quality of life. 

  • Humana is recruiting a team of multi-disciplinary professionals with expertise in trauma-responsive care, clinical social work, and holistic approaches. The program will be staffed 24 hours a day, seven days a week, and provide consistent supervision to participants. Supports extend to the community, inclusive of caregiver engagement, and supportive transition planning where a collaborative approach will work towards providing program participants with positive, prosocial community engagement, aimed to strengthen life skills and transition beyond the program.

Ideal candidates have:

  • Respectful, empathetic, and non-biased communication skills
  • Sound judgement and discernment in decision-making
  • Conflict resolution and crisis de-escalation and management skills
  • Experience establishing and maintaining therapeutic relationships with youth
  • Proficiency in the use of computers and various software applications

Clinical Case Coordinator (2 Full-time positions rotating days and evenings)

  • Independently complete assessments, review clinical notes, and integrate all available information into the development and documentation of plans of care that include appropriate therapeutic modalities
  • Work collaboratively with the rest of the Hunter House team ensuring plans of care are implemented, and all youth receive appropriate formal and informal supports while in program
  • Engage/communicate with families and external partners as appropriate
  • Requires registration with one of the professional colleges that allows use of the controlled act of psychotherapy (preferably the Ontario College of Social Workers and Social Service Workers).  A relevant master’s degree (in Social Work or equivalent) may be considered as an alternative to a professional designation depending on amount and relevance of previous experience.

Compensation:  Rate of pay for these positions is currently under review.   Employees at Humana receive paid vacation time off and a comprehensive benefits package including RRSP matching after 3 months of employment.

Additional Mandatory Requirements for all positions:

  • Legally entitled to work in Canada.
  • Valid Class G (full) driver’s license with a 3-year Uncertified Driver Record.
  • Fluent speaking, writing and reading in English. Fluency in other languages is an asset.
  • Satisfactory, current (within 6 months), Police Vulnerable Sector Check for employment purposes.
  • Physical Health Assessment: verification for up-to-date immunizations, COVID vaccination status (2 doses), and ability to meet the physical requirements of the position (mandatory).
  • Current Standard First Aid & CPR.
