Title:               Program Assistant, Community & Supportive Care

Classification: 30 hrs per week (Four 7.5-hour shifts), On-site

Wage Range:  $24.00 To $26.00 per hour

Reporting to: Director, Community & Supportive Care

POSITION SUMMARY:

St. Joseph’s Hospice is a faith-based organization that provides quality of life at the end of life, offering compassionate holistic care and support to individuals living with a terminal illness. Support is extended to their family members and caregivers, and to those grieving the loss of a loved one. Hospice programs and services are at no cost to those we support and are available irrespective of age, gender, religion, race, ethnicity or economic status.

Whether in the community, our wellness centre or our 10-suite residence, St. Joseph’s Hospice provides specialized programs and services with a person-centred approach. Our competent and committed Hospice staff and volunteers work together as a team with other community partners and service providers to provide full support and quality hospice palliative care.

Working with an interdisciplinary team,the Program Assistant, Community & Supportive Caresupports the seamless delivery of care within the Hospice, with a particular focus on providing a welcoming and supportive environment for clients, volunteers, and visitors. This important function will support the clients and volunteers of St Joseph’s Hospice through scheduling and providing access to services and activities offered in our Hospice Home and community. Additional duties include providing administrative and data support associated with program delivery.

We are pleased to offer the Defined Benefit Pension Plan HOOPP, (Healthcare of Ontario Pension Plan) from first day of employment on a voluntary basis and Insured Benefits (Health, Dental, Life Insurance) after three months employment.

Working closely with the Program Coordinator, the Program Assistant will:

  • Provide client inquiry follow-up
  • Schedule client intakes
  • Schedule clients for services and communicate in regard to changes, updates and cancelations
  • Maintain computerized client records and database entries related to bereavement programs
  • Register clients for all appropriate services and programs
  • Maintain accurate list of client participation in services and programs
  • Email/mail Monthly calendar of programs and services
  • Ensure client files are created and maintained
  • Organize and prepare Wellness Centre including maintenance of treatment rooms, and laundry upkeep
  • Provide comprehensive meeting room scheduling and support including logistical, technological and catering requirements
  • Ensure accuracy of data entry for reporting purposes
  • Respond to website inquiries in a timely manner
  • Provide administrative support to the Community & Supportive Care staff
  • Work in collaboration with the Program Coordinator on client intakes, program planning and special events
  • Organize activities and effectively communicate with confidence and professionalism
  • Ensure volunteers work in a safe, healthy, and supportive environment in accordance with all appropriate legislation and regulationsOther duties as assigned from time to time.

Qualifications & Requirements:

Post-secondary diploma Office Administration or similar

2+ years relevant work experience

Office administration experience in a health care setting is preferred

  • Strong verbal and written communication skills
  • High degree of attention to detail and results focused
  • Strong organizational skills and the ability to work in a fast-paced environment
  • Demonstrated time management skills and ability to prioritize work
  • Demonstrated proficiency with Microsoft Office and various software
  • Experience managing schedules and supporting multiple people
  • Experience working with volunteers
  • Physically able to move furniture and lift up to 40 lbs
  • Ability to be onsite 4 days per week and attend occasional evening/weekend events
    • Demonstrated relationship management skills consistent with the Mission, Vision, and Values of St. Joseph’s Hospice of London.
    • Police Records check for working with vulnerable persons
