Social Worker/Social Service Worker

Under the direction of the Manager of Clinical Services, the incumbent will work with persons living with Alzheimer’s disease or other dementias (ADOD), including Mild Cognitive Impairment (MCI), or other memory complaints. The Registered Social Worker/Social Service Worker will work with the individual, their care partners, and community resource agencies to ensure access to information, support, and education that assists them on their journey with the disease. The goal is to provide needed support, build effective and efficient utilization of community resources, promote links with primary care and community resource agencies; reduce care partner crisis situations and thereby reduce care partner burden.

Dementia Resource Consultant

Under the direction of the Manager of Community Clinical Services, this position serves as the initial point of contact for hospital Emergency Department (ED) and Home and Community Care Support Services staff and people living with dementia and their care partners who enter the hospital Emergency Department. These individuals may become clients of Alzheimer Society Southwest Partners (AlzSWP), including persons living with dementia and/or care partners.

In-home Recreation Coordinator

The In-home Recreation Coordinator will assess, plan, implement and evaluate individualized recreation opportunities for persons living with dementia (PLWD) in their home. There will be requirements of direct contact with clients to run programs within the home following safety procedures. Clients participating in the In-home Recreation program are typically in the early to mid-stages of dementia. The programs are designed to provide cognitive stimulation and social interaction for the person living with dementia. This time spent with the person living with dementia (virtually or in home) will offer respite to the care partners. The In-home Recreation Program aims to provide the opportunity to build a support network, get in touch with the community, and obtain further knowledge of Alzheimer Society resources.

Reporting to the Vice President Philanthropy, and working closely with the CEO, the Director, Marketing and Communications is a key member of the Foundation’s Executive Team, leading a small, high-performing team.
